Sponsor Affiliate Accounts

Special Note

If you are an affiliate wanting to obtain an account, you will need a current member of UTC faculty or staff to sponsor your account.

Directions

1. Open your preferred web browser to: Affiliate Service Request
    
2. Under the 'Username' field, enter your UTC ID (ex. abc123)
    
3. Under the 'Password' field, enter your UTC password
    
4. Select 'Log In'
    
5. Under the 'Phone Number' field, enter your phone number (ex. 423-425-4000)
    
6. Under the 'How many affiliate...' field, enter the number of accounts you need created.
   • If you entered a number higher than 1, enter the group name
      
7. Under the 'Arrival Date' field, enter the date you expect the Affiliate(s) to arrive (ex. 12/31/2000)
    
8. Under the 'Departure Date' field, enter the date you expect the Affiliate(s) to depart (ex. 12/31/2000)
    
9. Select 'Next'
    
10. Under the 'First Name' field, enter the Affiliate's first name (ex. Jane)
     
11. Under the 'Last Name' field, enter the Affiliate's last name (ex. Smith)
     
12. Under the 'Email Address' field, enter the Affiliate's email address (ex. janesmith@example.com)
     
13. Under the 'Phone Number' field, enter the Affiliate's phone number (ex. 423-425-4000)
     
14. Under the 'Date of Birth' field, enter the Affiliate's date of birth (ex. 12/31/2000)
     
15. Answer the 'Is the affiliate...' question

• If 'yes' is selected, enter the UTC address

1. Select 'Next'
    
2. Repeat steps 10 - 16 according to the number entered in step 6.
    
3. Answer the 'Resource(s) Required' question

• If 'UTC Learn' is selected, answer the 'Ferpa Training Required?' question

4. (optional) Answer the remaining questions
    
5. Select 'Submit'
    

Once the form has been submitted, an IT support ticket will be entered in your name. You should receive an email confirmation within 10 minutes. You will receive another email when the request has been fulfilled or if any additional information is required.
