If you are an affiliate wanting to obtain an account, you will need a current member of UTC faculty or staff to sponsor your account.
Please allow 2-3 business days for accounts to be created, time may vary based on services requested.
Directions
- Open your preferred web browser to: Affiliate Account Creation Workflow
- Under the 'Username' field, enter your UTC ID (ex. abc123)
- Under the 'Password' field, enter your UTC password
- Select 'Log In'
- Ensure the 'Requestor' and 'Requestor's Department' accurately show your account and department.
- Under the 'More than one affiliate account?' field, Choose “Yes” if submitting on behalf of multiple affiliates or a group. Choose “No” if submitting for a single individual.
- If requesting affiliate accounts for multiple individuals, please use the spreadsheet template provided on the service request page and attach to this form. All accounts must need the same access.
- Answer the 'How is this person affiliated with UTC?' or 'Affiliate Group Name' field

- Under the 'First Name' field, enter the Affiliate's first name (ex. Jane)
- Under the 'Last Name' field, enter the Affiliate's last name (ex. Smith)
- Under the 'Affiliate Date of Birth' field, enter the Affiliate's date of birth (ex. 12/31/2000)
- Under the 'Affiliate Email Address' field, enter the Affiliate's email address (ex. janesmith@example.com)
- Under the 'Affiliate Phone Number' field, enter the Affiliate's phone number (ex. 423-425-4000)
- Under the 'Arrival Date' field, enter the date you expect the Affiliate(s) to arrive (ex. 12/31/2000)
- Under the 'Departure Date' field, enter the date you expect the Affiliate(s) to depart (ex. 12/31/2000)
- Answer the 'Additional Resources Required' question
- Provide additional comments or instructions
- (For multiple accounts) Attach the spreadsheet template with the group member information
- Select 'Submit'
Once the form has been submitted, an IT support ticket will be entered in your name. You should receive an email confirmation within 10 minutes. You will receive another email when the request has been fulfilled or if any additional information is required.
